Survey Rules

By voluntarily participating in this survey, you consent to our full entry rules below.

Contest Rules:

1. No purchase necessary
   1. Participation is free but you must complete the survey to be eligible for entry. Only one survey per individual is permitted. Submitting more than one survey or completing both surveys (client and non-client surveys) voids all participation and entry, and the individual will be excluded from prize drawing.
2. Geographic area of sweepstakes and/or who is eligible to participate
   1. This contest is open to all reQuire clients or non-clients who receive monthly reQuire newsletters and whose permanent residence is in the US, including Alaska and Hawaii.
3. Opening and end dates
   1. Surveys must be completed between February 9, 2021 and March 4, 2021 4:59pm EST. Entrants must be identifiable by including their first and last name, and email address in the survey form prior to submitting.

5. Number of prizes, description of each, retail value and odds
   1. There is one prize with a retail value of $100.00 in the form of a Visa gift card. Odds of winning determined by number of entrants.
6. How winners are selected and when a determination of winners will be made
   1. A winner will be randomly selected from all entrants by March 5, 2021. The survey will be closed and/or no surveys submitted after March 4, 2021 4:59pm EST will be considered eligible for entry. 
7. Where and when a list of winners can be obtained
   1. The winner will be announced in the reQuire March newsletter. By submitting a survey, entrants consent to being identified and contacted. The winner will be contacted prior to the March newsletter being released to inform them, and receive the gift card electronically or via mail. The March newsletter will also be accessible online on reQuire’s website and will list the winner.
